- Copy of your driver’s license
- Vehicle Registration
- W9 Form (copy of your Social Security Card and work authorization documents)
- Driver Agreement signed (ask for one to email you)
- Driver’s Profile Form filled
- Your Certificate of Insurance Liability (with PECAS Services Inc as an additional insured
- $1,000,000 auto liability
- $100,000 Cargo coverage
